Service Provider Application Addendum - January 9, 2009

At the request of the Environmental Resources Coalition (ERC), the CLEANmp-West Program Administrator id requesting that all approved service providers submit a completed W-9 form.  If a service provider has elected to have payments remitted to their employer, the employer should complete and submit the W-9.  Completed W-9 forms should be sent to: SES, Inc.; CLEANmp-West Program Administrator; 6750 Antioch Road; Suite 305; Merriam, Kansas 66204.
